Manresa (Barcelona) Implements a Groundbreaking Model of Comprehensive Home Care Based on Alexia SISS

The SISS Manresa model is a pioneering project that has enabled the consolidation of the integrated care model in health and social services in the city of Manresa and its surrounding area, with ISERN Medical as the developer of Alexia SISS-the key technological platform behind this integration.

The conference "From Project to Model", held on June 6, 2025, in the auditorium of the Hospital Sant Joan de Déu in Manresa and at the Simulation Area of the Althaia Foundation, brought together professionals, institutions, and technology companies to share the evolution of the SISS Manresa model, which has grown from a pilot project into a fully consolidated system and a benchmark in the field of integrated care.

During the event, it was emphasized that this model is the result of joint efforts by several entities, including Althaia, ICS Catalunya Central, Sant Andreu Salut, the Manresa City Council, and the Departments of Health and of Social Rights and Inclusion of the Government of Catalonia.

This integration helps avoid duplication and enhances coordination between primary care professionals, home care services (SAD), and social services. The technology allows for the parameterization of risks such as nutrition, potential falls, or the need for hospital admission, which contributes to more efficient care delivery.

Alexia SISS is a platform that includes a mobile app, a web portal for professionals, and a portal accessible via TV or tablet from the patient's home-improving the user experience and coordination between healthcare and social service teams.

Positive Outcomes and Model Advancements

The results from the implementation of the SISS Manresa pilot have been very positive. Sixty people with varying levels of dependency, receiving home care, helped demonstrate the model's effectiveness. The results include:

  • 40% reduction in the time professionals spend on tasks.
  • 75% decrease in emergency room admissions.
  • 34% reduction in unnecessary visits to primary care centers (CAP).

A Person-Centered System

The goal of Alexia SISS is to help deliver a single, integrated care plan through a case management model, ensuring that each patient has specific contact persons for their care, both in the healthcare and social spheres. This initiative has been made possible thanks to effective institutional collaboration, which has successfully integrated technology with human resources to provide more personalized and efficient care.
